Initial OPT 提交文件要求

请问有人知道F-1 VISA页面是必须提交的么？I-765的Instruction写着；
If you were not previously issued an EAD, you must submit a copy of a government-issued identity document (such as a passport) showing your picture, name, and date of birth; a birth certificate with photo ID; a visa issued by a foreign consulate; or a national ID document with photo and/or fingerprint.
所以应该不是必须的吧。我看了不同学校的instruction不一定都写着需要。

还有一个问题是previous I-20。I-765的Instruction写着；
Use Part 6. Additional Information to provide all previously used SEVIS numbers and evidence of any previously authorized CPT or OPT and the academic level at which it was authorized.
如果有一些旧I-20是因为Extension of Stay，但SEVIS号都一样的话还需要提交么？

多交反正不会错

第一个问题，不需要，只用提交护照信息那一页就可以。
第二个问题不是很清楚，但如果是我我会提交。

如果是F1入境，F1 Visa + i94 最好都要交，以证明F1 身份，
如果是转成F1，就没有F1 Visa和i94, 就交原始Visa + i94 +  i797 A.


所有的I20 最好都有提交，以证明连续性。


学校一般有checklist, 如果没有查看其他学校的check list 也行，要求一般是一样的。
