申请stat的那个Interest Form是啥意思呀？

OSU他家stat的prospective students--graduate--application information之后的这个是啥意思呀？
Interest Form: Statistics and Biostatistics Graduate Programs
To express interest in graduate programs in STATISTICS or the PHD IN BIOSTATISTICS, use one of the following means of contact. At a minimum, we need your mailing address, email address, phone number, and which program you are interested in. Preferably, include all the information in the form below.
