Fee Payment Form在哪里呀？

邮寄材料的时候不是需要填写一个Office of Admissions Document ID Sheet和材料一起邮寄么？我看到这个ID Sheet 里面有这样一句话呀：To submit your application fee by credit card, check or money order, please print and mail Office of Admissions Application Fee Payment Form with this document identification sheet.

我想请问这个Fee Payment Form在哪里找的到呀？哪位童鞋可以给个链接嘛
