关于FICA 退税的地址问题

请问地里的各位大大们，有没有最近刚交FICA退税的？ 我现在不太清楚到底要寄到哪个地址，我现在在网上找到多个地址，请求大家帮帮我谢谢！！
我在第一个地方看到邮寄的地址如下: https://www.irs.gov/individuals/ ... and-self-employment

上面说是 “File Form 843 (with attachments) with the IRS office where your employer's Forms 941 returns were filed. You can locate the IRS office where your employer files his Form 941 by going to Where to File Tax Returns.”

我点进去以后 找到了 Mailing Addresses for Forms 941， 但是我发现有好几种 Form 941:

Form 941 Employer's Quarterly Federal Tax Return
Form 941-PR Employer's Quarterly Federal Tax Return (Puerto Rican Version)       
Form 941-SS Employer's Quarterly Federal Tax Return - American Samoa, Guam, the Commonwealth of the Northern Mariana Islands, and the Virgin Islands       
Form 941-SS which does not include a payment
Form 941-SS which includes a payment
Form 941-X Adjusted Employer's Quarterly Federal Tax Return        Form 941-X

请问大家是要看哪种？ 还是问自己的HR? 我问了我的HR说他们不处理Form 941... 但是我猜测大多数美国公司是不是都是第一种？

点进去form941 Employer's Quarterly Federal Tax Return 以后， 里面的地址也有两种： Mail return without payment 和 Mail return with payment ， 请问大家都是选择哪种啊？

我还看到另一个版本的地址：在form 843的instruction里说要寄到Pub.519中提及的地址: Department of the Treasury, Internal Revenue Service Center, Ogden, UT 84201-0038

这个地址跟我上面 Mailing Addresses for Forms 941 里的第二个地址差不多

如果有朋友最近file了，可以麻烦告诉我一下到底是寄到哪个地址吗！！十万个感谢！！

我不是4月份file的按照一个攻略上寄的，就是UT这个地址。但是至今没音儿。有些大学的ISSO网站上说寄Austin的地址，我打算试试看。我也觉得楼主说的这个对照表，至少对我们FICA退税843+8316的非常非常的confusing

你好，我最后也是寄去UT的地址，但是也是还没收到任何消息，但是我看别人都说这个部门工作效率很慢，如果你怕不对 另外个地址也可以寄

猛然看到这个！我是今年4月寄到UT的地址的，毫无音讯。想着到10月再寄一份吧。心塞的是我公司不看到IRS的结果，现在还在继续扣着FICA，明年还得再来一次。

请问楼主有消息吗 我去年九月寄到UT的地址也是毫无音讯
