First, log in to your account on Net Benefits:
Go to the “rollover” quick link next to the plan you want to roll your IRA to.
Click on “Roll Over to Plan”
“Do you have a check?” Click yes. (Even though you don’t.)
Confirm your contact information.
“What company manages your Plan?” Enter “Fidelity” and your IRA account number. (e.g. Fidelity Y1234567)
“What type of Plan?” Click “Rollover IRA” or “Traditional IRA”
“Do You Know Your Contribution Type?” Click Yes.
Enter the current value of your IRA and confirm.
Click “I agree” to confirm the roll over. (You will not need to print a deposit slip.)
Second, call the Fidelity IRA department: 800-544-5373
Request a rollover from your Ira to your Fidelity plan.
Advise the representative that you have contacted the 401K department and entered in the deposit information.
Request that your plan number be placed on the check.
Request the check be mailed directly to Fidelity Investments PO BOX 771063 Cincinnati OH 45277-1063
