询问各位申TAMU的，有关Bacterial Meningitis Rqmts

今年TAMU的网申貌似突然多了这么一项。。。在custom question那一栏里，上面写的要求如下：
Senate Bill 1107 mandates that all entering students (both domestic and international) at an institution of higher education (public or private) must provide proof of bacterial meningitis vaccination not later than the 10th day before the first day of the semester beginning January 2012. Students may provide one of the following:
Proof of vaccination (form signed by doctor, shot record, or official school record noting vaccination)
An affidavit signed by a physician noting that the vaccination required would be injurious to the health and well–being of the student, or
An affidavit for exemption from the immunization for reasons of conscience Formerly enrolled students who leave the university for a fall or spring term must provide proof of immunization prior to returning to the university. More information can be found on the admissions website.

我想问一下，这个疫苗证明从哪里开啊？自己之前的疫苗记录找不到了，那需要去哪个部门开具呢？是校医院还是当地的出入境检疫中心？谢谢各位版友了！
