This is a follow-up email notice to remind you of your right to submit a Claim Form to determine your eligibility for a settlement  payment. You are receiving this notice because we previously sent you an email between October 4, 2024, and October 9, 2024, and we have not yet received a Claim Form submission from you.
For the Settlement Administrator to determine your eligibility for a settlement payment, you must submit a completed Claim Form online, via email, or by mail postmarked no later than January 2, 2025. Submitting a claim is the only way to determine your eligibility for a settlement payment.
A Claim Form may be submitted in one of the following ways:

• Online: The fastest, most secure, and preferred way for you to submit your Claim Form is using the claims filing module on the website

• Email: Complete, sign, and scan your paper Claim Form and email it to the Settlement Administrator at info@DOJPERMClaims.com.

• U.S. Mail: Send the completed and signed Claim Form to:
