OPT Extension Form I-765的几个问题

ISSS电话打不通，邮件也没回 (cannot find detailed instructions for the following two items in my school's website, nor was anything explained on USCIS site)，比较着急，在这里问一下。

• item 11 Have you ever before applied for employment authorization from USCIS里面的日期是填上回申请opt的哪个日期？我手头有Form I-797C上的received date, notice date, 还有i797的approval date和ead卡的start date.  [UPDATE: I found the following instruction on Harvard ISSS website and on NYU ISSS site: "indicate the dates of approval or denial", while the instruction on UMICHI says "write the start and end date of the EAD card", and on UW-Platt "list the date on which you applied (receipt date on I-797C Notice of Action)", and on Stanford "find the Received Date in the Notice of Action letter (I-797C)". Orz....]
• item 17 list your degree是写master's就可以了，还是要写具体的master of science in engineering in [major]这样子的？如果要写major的话，我transcript上的major和i20上的major不完全一样（前一个是后一个的一个专业方向）写哪个？[UPDATE: according to UW-Platt, I should list my "degree (level and major) as it appears in SEVIS" while UChicago says "list the numerical CIP Code for your degree (this information can be found on the top of page #3 of your I-20)"]

Just got back from my ISSS adviser. In her email, she circles receipt date for item 11 and for item 17, she says "What you feel best answers the question" <---- =____=WTF?! I guess I can draw a conclusion that it doesn't really matter whatever I put there.

吐血了，又出新问题了。我给A公司的subsidiary B公司工作，但是我的paycheck都是走A公司的。Request I20 for STEM OPT时我问ISSS回说不确定but don't think it matters,问HR她不回邮件，我等不及了考虑到上次申请H1B填的是B公司, 所以地址和公司名字填的是B公司。但是刚又收到HR通知Employer's name as listed in e-verify应该填A公司。现Form I-765 item 17我填A公司的话，那和I20上employer name不match了，现在重新request I20来不及了。要是我这样把材料寄出去呢，还是该怎么办？

另外现在虽然我给B公司工作，我的工作地点是和B公司主要地址相隔一条街的A公司楼，我I20上employer's address写的是和H1B申请一样的B公司主要地址。但是HR现在给我的地址是我现在工作的A公司楼地址，但是这和http://www.uscis.gov/e-verify/about-program/e-verify-employers-search-tool搜索出来的A公司地址也不是同一个。

我学校在东岸，人在西岸，ISSS今天已经关门，现在真是连问的人都没有了。

Mailed my application on Sat 5/31 via Fedex express and it was delivered on the morning of Monday 6/2. Meanwhile, emailed my ISSS adviser to ask about the company name & address issue. She got back to me on 6/2 suggesting me contact her once I have a receipt number for my application and then we can see what is best by contacting the service center at that time in regards to what to update and/or submit to them.
Got email/text msg confirmation from USCIS on Thur 6/5. Received the hardcopy I797C via mail on Sun 6/8, which states that the received date is 6/2 and the receipt date is 6/4.

Forwarded the USCIS receipt email to my ISSS adviser. She sent an updated I-20 and email notification to the California Service Center, and got a response from USCIS on 6/12: "The application has not yet been assigned to an officer. I will interfile a copy of this email and its attachments."

问题一，日期写第一个OPT开始的日期，就是EAD卡的日期。问题二，学位写毕业证上具体的学位，就是of后面的内容，写不下也没关系，因为同时附上了一份成绩单或学位证。希望有帮助

第一个问题 是 你收到纸质 notice上的 日期。 第二个问题你学要打电话到学校的international office 去问清楚

which date on 纸质 notice? I have their received date, notice date, approval date on form I797C & Form I797.

我们学校的ISSS准备的表格直接没有写这个日期。另外问一下17个月是怎么计算的，比如05/30/2014开始，那么结束是不是10/29/2015？

学校小秘给准备的i20和表格好多错误。。

The ISSS at my school put 6/10/2014 - 11/10/2015 on my OPT extension request i20. I'm a little bit concerned that ISSS counted one extra day for my OPT extension, b/c my opt date is 6/10/2013 - 6/9/2014.

这个问题我也问过学校了，他们说时间是系统生成的，延期的时候是6/10到11/10，我原来的opt也是类似的6/10-6/9，看来这个不会有问题。

我之前回复的好像不对，问了上一届的同学他们的时间少一天，比如6月10号到11月9号。LZ还认识其他同学吗？可以再确认一下

感觉这个问题还是挺普遍的，我另外发了个贴专门讨论，方便今后大家查询。OPT Extension时间问题
