* This is the time to brag, so don't be shy.
* Talk about What. Find org level initiatives/goals, figure out how your projects/accomplishments are tied to them. Put them in logical categories, not just a flat list of random things you did.
* Also talk about How and Why. Companies often have culture they want to foster, many have documents describing what an ideal employee looks like. Describe how you demonstrated these attributes throughout the year.
* Find the role profiles (for your current and the next level), understand the expectations, especially the differences among various levels if you’re seeking a promotion. It’s quite possible that your manager needs to present some sort of promo doc (maybe only a few bullet points, maybe long; maybe verbal, maybe in writing), it’s much easier for him/her if you write the first draft.
For the coming year -
* Start writing your review doc at the beginning of the cycle. What you want to accomplish, kind of like a plan. Fill in the blanks and make adjustments as the year goes by, so you don’t have to recall what you did months later.
* Share your success with your manager throughout the year and ask for his/her feedback.
* Summarize them again in the review doc at the end - your manager is busy, he/she may be impressed by your accomplishment at the moment, but probably won’t remember everything months later.
